Ministry of Health (MINSA) Resolution No. 0864, issued on September 24, 2026, took effect upon publication in Official Gazette No. 30630 on October 9. The resolution regulates the process of entry into the Unified Health Facility Licensing System and repeals Resolution No. 409 of 2025. Its scope includes public and private healthcare establishments, mobile units and external support services, including sterilization centers and hospital laundries.

Article 3 requires facilities and support services already operating when the resolution was promulgated to enter the system no later than May 31, 2028. The deadline concerns entry into the procedure, rather than the issuance of the certificate. Holders of a valid Health Operating Permit from the Technical Health Council must enter the system before that permit expires and meet the requirements: the exception changes the timetable but does not remove the licensing obligation.

The procedure comprises registration, plan approval, submission of the required documentation, standards verification and certification. Inspections assess infrastructure, furnishings, biomedical equipment and staff academic credentials according to the services offered. For new facilities, articles 14 and 20 require registration and formal initiation of the procedure with complete documentation; they allow operations to begin to enable inspection, without this constituting certification. Issuance by the General Directorate of Public Health confirms that the requirements have been met and formally authorizes the facility to operate.

For clinics, hospitals, laboratories and support providers, the framework covers opening and continued operation. Article 19 establishes renewable five-year certificates, which must be displayed in a visible, publicly accessible place. Renewal must be requested three months in advance. Article 23 distinguishes between maintaining licensed conditions and modifying them, depending on changes to infrastructure, service offerings, complexity or other authorized conditions.

Relocation requires a new certificate and a new assessment: the facility's existing certificate does not cover another location, under article 26. Administrative changes to the legal entity name, trade name or legal representative require an update request within the following thirty calendar days, supported by documentation. These obligations affect planning for openings, relocations and reorganizations in the sector. Obtaining the certificate remains subject to each facility's compliance with requirements and standards, as well as timely entry into the system.
